About the Role
We have an opportunity for a Registered Nurses to provide in-home and community-based care across Far East Gippsland region.
As a Community Nurse, you will:
- Deliver personalised nursing care in clients’ homes.
- Perform clinical assessments and develop individualised care plans
- Monitor chronic conditions and provide wound care, medication management, post-acute care, and health education
- Work autonomously while also collaborating with a multidisciplinary team
- Build strong relationships with clients, families, GPs, and allied health professionals
- Empower clients to remain independent and improve their quality of life
Requirements
- Bachelor of Nursing or Diploma of Nursing
- Current AHPRA registration
- Valid driver’s license
- Willingness to travel locally for home visits
- Current National Police Check and NDIS Worker Screening Check (or willingness to obtain)
- COVID-19 and flu vaccinations
Desirable
- Experience in community, primary health, or aged care nursing
- Strong knowledge of chronic disease management, palliative care, and dementia support
- Excellent communication, time management, and documentation skills
- Ability to work independently and manage a mobile caseload
- Passion for delivering care in a person-centred and respectful manner
