Diabetes Educator Registered Nurse – Regional Victoria (3 Hours drive from Melbourne)

Are you passionate about improving health outcomes for people living with diabetes in rural communities?
An exciting opportunity exists for an experienced and motivated Diabetes Educator to join a dedicated and supportive Community Health team in regional Victoria, approximately three hours’ drive from Melbourne.
This position plays a pivotal role in providing person-centred diabetes education and clinical care across acute, community and aged care settings. You’ll work both autonomously and as part of a multidisciplinary team to enhance access to care, promote self-management, and improve quality of life for consumers across the region.

Key Responsibilities
- Deliver high-quality, evidence-based diabetes education and clinical support to individuals and groups across acute, community and aged care settings.
- Collaborate closely with General Practitioners, nurses, allied health, and community services to ensure coordinated, person-centred care.
- Conduct comprehensive assessments, develop care plans, and support consumer self-management strategies.
- Provide education on insulin administration, glucose monitoring, sick day management and lifestyle modification.
- Utilise telehealth and digital tools to reach clients across the catchment area.
- Participate in health promotion, risk management, and quality improvement initiatives.
- Maintain accurate documentation and comply with all relevant legislation, policies, and professional standards.
Essential Criteria
- Current AHPRA registration as a Registered Nurse (Division 1).
- Graduate Certificate in Diabetes Education (or working towards completion).
- Eligible for membership with the Australian Diabetes Educators Association (ADEA).
- Demonstrated experience in diabetes education within a community or primary health setting.
- Proven ability to work both independently and collaboratively within a multidisciplinary team.
- Excellent communication, interpersonal and organisational skills.
- Proficiency in Microsoft Office and clinical documentation systems.
- Current Victorian Driver’s Licence, Police Check, and Working with Children Check (or willingness to obtain).
Desirable
- Experience working in rural or regional health settings.
- Knowledge of culturally safe practice and chronic disease prevention strategies.
