We’re seeking a Client Care Coordinator with a strong background in healthcare to help deliver high-quality, person-centred support for our Home Care clients. Whether you’re a nurse, allied health professional, or experienced in aged care coordination, your healthcare knowledge will be invaluable in ensuring our clients receive the best possible care.
About the Role:
You’ll be managing an existing portfolio of home care clients, working closely with them and their families to develop individualised care plans, coordinate services, and monitor outcomes. Collaborating with our broader State Team, you’ll help deliver tailored care that supports independence and well-being.
In this role, you’ll:
- Deliver clinical and non-clinical care coordination to ensure best outcomes for clients.
- Work with clients and families to co-design care plans that reflect their health goals and personal needs.
- Support client independence by ensuring services are safe, appropriate, and timely.
- Liaise with nurses, GPs, and allied health professionals to ensure continuity of care.
- Monitor and manage care budgets in accordance with Home Care Package (HCP) program guidelines.
- Coordinate client intake, onboarding, and ongoing communication.
You’ll bring:
- Background in healthcare: nursing, allied health, aged care or community care.
- Understanding of Home Care Packages and the Aged Care Quality Standards (or a willingness to learn).
- Strong communication and relationship-building skills.
- Empathy, emotional intelligence, and a genuine passion for client-centred care.
- Ability to manage changing needs and priorities in a fast-paced environment.

Qualifications & Experience:
- Relevant qualifications or experience in healthcare industry or similar
- Strong understanding or Aged Care and/or other health related services
- Experience and success in working in a customer focused service business.
- Current or willing to obtain National Police check
