About the Role

We have an opportunity for a Registered Nurses to provide in-home and community-based care across Far East Gippsland region.

As a Community Nurse, you will:

• Deliver personalised nursing care in clients’ homes.
• Perform clinical assessments and develop individualised care plans
• Monitor chronic conditions and provide wound care, medication management, post-acute care, and health education
• Work autonomously while also collaborating with a multidisciplinary team
• Build strong relationships with clients, families, GPs, and allied health professionals
• Empower clients to remain independent and improve their quality of life

Requirements

• Bachelor of Nursing or Diploma of Nursing
• Current AHPRA registration
• Valid driver’s license
• Willingness to travel locally for home visits
• Current National Police Check and NDIS Worker Screening Check (or willingness to obtain)
• COVID-19 and flu vaccinations

Desirable

• Experience in community, primary health, or aged care nursing
• Strong knowledge of chronic disease management, palliative care, and dementia support
• Excellent communication, time management, and documentation skills
• Ability to work independently and manage a mobile caseload
• Passion for delivering care in a person-centred and respectful manner
